Thus Dr. Lawrence Weed began his 1971 Grand Rounds presentation at Emery University on problems with the medical record.  The issues that Dr. Weed, the father of the Problem Oriented Medical Record, emphasized in this presentation are many of the same critical problems that we grapple with today and that contribute to preventable medical mistakes that kill more than 100,000 patients annually.

This is a compelling video. If you are a healthcare leader—physician or lay executive—who cares about quality, safety and making a difference, this is a must-see presentation.
